Gauteng's first MitraClip Implants a 'gamechanger' for heart valve treatment
The Charlotte Maxeke Johannesburg Academic Hospital (CMJAH) has performed the first successful MitraClip implants on three State-funded patients in the province.
The minimally invasive procedure is designed to treat Mitral Regurgitation, a heart condition where the mitral valve does not close properly, causing blood to leak backwards into the heart.
The Gauteng Health Department explained that the MitralClip itself is a “minute clip passed through a catheter, which aids the mitral valve in sealing properly, subsequently restoring normal blood flow through the heart”.
Gauteng MEC for Health and Wellness, Nomantu Nkomo-Ralehoko welcomed the successful completion of the procedures, which were carried out last week.
“This procedure is but an example of the many inroads we continue to make in the healthcare system through collaboration between the public and private sector.
“We have the advantage of academic hospitals, which boasts leading experts in many fields locally and internationally. This allows us to pioneer new ways of improving patient care,” the MEC said.
Director of the Cardiac Catheterisation Laboratory and Interventional Cardiologist at CMJAH, Dr Arthur Mutyaba, who was part of the team performing the procedures, said that with this new capability, more patients living with the condition and ineligible for open-heart surgery can now be assisted.
“With this procedure now accessible, we are able to help these patients get back to a normal life without having to expose them to the risk that open heart surgery would have disposed them to,” Mutyaba said.
The department revealed that, to date, at least 15 MitraClip procedures have been completed in South Africa, all performed in public institutions.
“The Charlotte Maxeke procedure marks the fourth round of MitraClip implants in South Africa, following the first 12 successful procedures performed at Groote Schuur and Tygerberg hospitals in Cape Town.
“What makes this program unique is that both public and private patients have access to this cutting-edge therapy through these leading academic hospitals in the country. At present, MitraClip therapy is not yet available in private healthcare facilities, making these institutions the sole centres for this life-changing intervention,” the department said.
Game changing solution
The Gauteng Health Department described that the procedure as a “game changer” which marks a “significant advancement in minimally invasive heart valve therapy” in both the private and public sector.
“The procedure’s key benefits include drastically reduced recovery times, often just one or two days post-insertion, making it a game-changer for elderly or high-risk mitral regurgitation patients who are unsuitable candidates for open heart surgery.
“With its availability through academic hospitals and the ongoing tracking of patient outcomes, this pioneering therapy is set to transform the treatment landscape for mitral regurgitation in South Africa,” the department said.
The successful procedures will now be recorded on the national registry to track advancements in treatments in South Africa.
“What makes the procedures even more unique is that, despite most of the patients having full medical insurance, a decision was made to perform these procedures in a controlled academic hospital environment.
“This approach ensures that all cases are recorded in a national registry, allowing for the tracking of long-term outcomes and contributing to the advancement of structural heart disease treatment in South Africa,” the department said. – SAnews.gov.za

The Portfolio Committee on Health received a comprehensive briefing from the Minister of Health, Dr Aaron Motsoaledi, regarding the recent withdrawal of funding from the United States President's Emergency Plan for AIDS Relief (PEPFAR).
During the briefing, Minister Motsoaledi highlighted PEPFAR's critical role in supporting various health initiatives, stating that approximately 7.8 million South Africans are living with HIV/AIDS, the highest number globally. The Minister emphasised that PEPFAR funding has been instrumental in addressing the needs of the 27 high-burden districts, which were selected for support based on specific criteria.
During the discussions on the US withdrawal of PEPFAR funding, committee members had questions about the immediate and long-term implications of this decision. Members expressed concern that the suspension of funding could lead to significant disruptions in HIV prevention and treatment services, particularly in these 27 high-burden districts that rely heavily on PEPFAR support.
The committee also questioned the contingency plans that have been put in place for maintaining service delivery, the potential impact on patient care, and how the department intends to support the healthcare workforce affected by the funding cut.
The committee sought clarity on how the government plans to address the anticipated shortfall and the measures that will be put in place to ensure that those living with HIV continue to receive the care they need.
Committee members emphasised the need for South Africa to strengthen its sovereignty in health matters and reduce reliance on foreign funding. They also highlighted that the withdrawal of PEPFAR funding serves as a wake-up call for the country to develop sustainable healthcare solutions independent of external influences.
Members urged the Minister to prioritise local production of essential medications, such as antiretrovirals (ARV), and to explore innovative funding mechanisms that could support health initiatives without depending on foreign aid. Clarifying the situation regarding ARV treatment, the Minister reiterated that South Africa relies primarily on its own budget for ARVs, with approximately 90% of the medication being locally funded.
The briefing also covered issues related to the lack of employment for healthcare professionals, including doctors, with members expressing deep concern about the rising numbers of qualified healthcare professionals who are unable to find employment in the public sector. The committee questioned the long-term implications of this trend on the healthcare sector, noting that an adequate workforce is essential for delivering effective health services, especially in underserved areas.
Committee members questioned the Minister about the current vacancy rates for medical positions and urged him to outline specific strategies for integrating newly qualified doctors into the healthcare system.
Suggestions made included the need for a streamlined recruitment process, the development of internship programmes that guarantee employment upon completion, and collaboration with provincial health departments to prioritise hiring in areas with the greatest need. The committee urged the Minister to explore strategies for absorbing newly qualified healthcare practitioners into the public healthcare system.

Merck Foundation (www.Merck-Foundation.com), the philanthropic arm of Merck KGaA Germany, conducted the 11th Edition of their annual conference, “Merck Foundation Africa Asia Luminary”, under the patronage of The President of Tanzania, and in partnership with the Government of Tanzania on 29th and 30th October, in Dar es Salaam, Tanzania. The conference was officially inaugurated by H.E. Dr. SAMIA SULUHU HASSAN, The President of the United Republic of Tanzania together with Prof. Dr. Frank Stangenberg-Haverkamp, Chairman of Merck Foundation Board of Trustees and Senator, Dr. Rasha Kelej, CEO of Merck Foundation, along with First Ladies of 15 Countries, who joined as the Guests of Honor and Keynote Speakers.
While inaugurating the conference, The President of United Republic of Tanzania, H.E Dr. SAMIA SULUHU HASSAN emphasized “I am delighted to host this prestigious conference in Tanzania. It is a great honor to inaugurate the conference alongside the First Ladies of Africa and Asia. During the 11th Edition of Merck Foundation Africa Asia Luminary, two important occasions were marked; the 7th Anniversary of Merck Foundation and 12 years of Merck Foundation's development programs that started in 2012.
On the first day of the conference, the Plenary Session of the Merck Foundation Africa Asia Luminary 2024 took place, featuring a high-level panel discussion with the participating First Ladies of Africa and Asia. Moreover, two high-level ministerial panel discussion were held with African Ministers and top healthcare experts from across the globe.
The Day 2 of the conference will have five key parallel medical and scientific sessions, covering topics such as oncology, diabetes and hypertension, fertility and reproductive care, and medical capacity building in other specialties like respiratory care, acute care, emergency pediatric and neonatal care, and more. Additionally, a community awareness session, Merck Foundation Health Media Training, will be conducted for African journalists. This session will emphasize the critical role of the media in influencing communities and driving cultural change, with regards to a wide range of social and health issues like Breaking Infertility Stigma, Supporting Girls' Education, Stopping GBV, Ending Child Marriage & FGM, Empowering Women, Diabetes and Hypertension Awareness.

As the wider African region experiences an upsurge in mpox cases, South Africa's multi-pronged efforts to bring the outbreak under control are bearing fruit. On 14 August 2024, World Health Organization (WHO) declared mpox a public health emergency of international concern and subsequently issued a set of temporary recommendations to scale up the emergency response.
South Africa had previously reported five unlinked mpox cases in 2022. Having confirmed zero cases in 2023, the country has experienced a resurgence of the infectious disease in recent months, reporting 25 laboratory-confirmed cases and three deaths between 8 May and 6 September 2024. These cases have been reported in Gauteng, Western Cape, and KwaZulu-Natal provinces, with local transmission suggested for 24 cases and one imported case.
However, there has been a significant change in the epidemiological data in recent weeks, with just one new case and no deaths reported since 2 August 2024, compared to 12 cases and three deaths in June, so far, the peak of the current outbreak.
“With the support of partners including WHO, we have made significant progress in our efforts to contain mpox in the country,” says Foster Mohale, spokesperson for South Africa's National Department of Health. “But we must not become complacent because viruses mutate from time to time. The recent detection of a new mpox case in Western Cape should be a strong reminder that we must all remain extremely vigilant.”
To coordinate its response to the outbreak, the National Department of Health formed an mpox Incident Management Team comprised of experts from national and provincial levels and partners including WHO. In June, with WHO support, health authorities conducted two remote webinars on clinical management of mpox cases, training over 500 health workers and public health professionals.
The department has also activated provincial response teams to implement priority interventions in hotspots, deployed rapid response teams to support community surveillance and case management, and intensified targeted public awareness campaigns.
In addition to providing technical support and guidelines on mpox treatment to help ensure standardized patient care, WHO has donated 35 treatment courses of tecovirimat, an antiviral that is used to treat severe mpox cases in conjunction with symptomatic and supportive care. The Organization is supporting efforts to stockpile an additional 30 treatment courses of the medication for rapid deployment should further cases arise.
South Africa is one of 14 countries in the African region that will be financially supported by the Africa Centres for Disease Control and Prevention (Africa CDC) and WHO from September 2024 until February 2025 to draw up and roll out plans, including vaccination strategies, to continue its fight against mpox. The Mpox Continental Preparedness and Response Plan for Africa will see an estimated US$ 45.5 of a US$ 600-million total budget allocated to South Africa, most of which will be spent on the logistics of rolling out vaccines.
“WHO remains committed to supporting health authorities in containing the mpox outbreak through coordinated efforts in treatment, surveillance, and public awareness,” says Dr Fabian Ndenzako, acting WHO Representative in South Africa. “I am pleased to see mpox control efforts targeting health workers, community members and key risk populations to prevent further transmission and manage this public health issue effectively.”

SA groundbreaking study transforms TB treatment policy globally
A groundbreaking initiative, BEAT-Tuberculosis, has become the world’s first programme in the battle against drug-resistant tuberculosis (DR-TB) for all population groups, influencing the international TB treatment policy of the World Health Organisation (WHO).
The principal investigator leading this study, Dr Francesca Conradie, explained that the study’s success in demonstrating the safety and effectiveness of the new short oral regimen in these key populations marks a pivotal step toward a more comprehensive and equitable healthcare strategy for TB.
“The BEAT-Tuberculosis study represents a groundbreaking approach in the fight against tuberculosis, particularly with its inclusive research methodology.
“By incorporating pregnant women and children into the clinical trials, the study has expanded the understanding of TB treatment’s efficacy and safety across broader demographics,” said Conradie.
The primary aim was to evaluate the effectiveness and safety of a novel shortened treatment regimen for DR-TB compared with the established standard of care.
The standard treatment consisted of a regimen of seven drugs administered over a minimum of nine months, in comparison to a regimen of four to five medications, which included newer agents like bedaquiline and delamanid, and was administered over only six months.
The BEAT-Tuberculosis clinical study was conducted at the Clinical HIV Research Unit (CHRU) Isango Lethemba TB Research Unit in the Eastern Cape and KwaZulu-Natal over the past six years.
The study began in 2019 and despite the formidable challenges posed by COVID-19, over 400 participants were enrolled across study sites in both provinces.
The study’s other objective was to build research capacity for DR-TB in the Eastern Cape and KwaZulu-Natal, through community engagement and training healthcare personnel in clinical research methodologies for TB treatment.
“This collaborative effort received crucial support from the National and Provincial Departments of Health that provided essential medication and laboratory infrastructure for safety and efficacy assessments,” said Conradie.
The local research study was made possible by the collaboration of the National Department of Health, funded by the United States Agency for International Development (USAID) and executed by the University of the Witwatersrand.
The results from the study have influenced the WHO policy on the treatment of TB, and at the same time put the Global South centre stage for driving improved treatment for all.
National Department of Health’s Chief Director of TB Control and Management, Professor Norbert Ndjeka, said that in 2022, an estimated 280 000 people developed TB in South Africa, while 54 000 died from the disease.
TB remains a threat to global public health and the second leading cause of death in South Africa.
“With this concerning observation in mind, we acknowledge the BEAT Tuberculosis project as an exemplary and concerted effort to advance TB treatment and research capabilities, poised to bring about transformative changes in RR-TB management worldwide.
“While the study was originally intended to inform local treatment guidelines, the results are now set to benefit patients everywhere.”
USAID Southern Africa Mission Director, Leslie Marbury, believes that the National Department of Health and his organisation have improved TB patient care worldwide.
Marbury said the organisation celebrates the 400 participants who made this trial possible and brought the world closer to ending TB.
“We also thank all BEAT Tuberculosis stakeholders, including the National Department of Health, the Eastern Cape and KwaZulu-Natal Health Departments, and local medical facilities, for their dedication and commitment to this globally recognised study,” she added.
TB symptoms
The most common symptoms of TB are a cough that lasts more than two weeks, weight loss, drenching night sweats and a fever.
Anyone with these symptoms is urged to go to their local clinic.
